Overview

In this form, you can choose one of the following: 

Individual – nominate a young person for your chosen award category.

Group – nominate a youth group for the Group Achievement Award 

If you’d like to make both an individual and a group nomination, please complete the form twice (once for each). 

If you'd like to nominate the same young person for a second award, fill the form in again. You can nominate one individual for a maximum of 2 awards.
